Whiplash in Children: Symptoms and When to Seek Urgent Care
Updated: Oct 19
A child with neck pain after a collision, fall, or sports impact needs an age-appropriate medical assessment. Whiplash describes an injury mechanism involving rapid head and neck movement; the label alone cannot tell you whether the injury is a muscle strain, concussion, or something more serious.
Children may not describe symptoms clearly, and changes can emerge after the event. Do not assume a child is uninjured because there is no bruise or because they initially seemed comfortable.

Emergency signs after a head or neck injury
Call 911 for breathing difficulty, a seizure, loss of consciousness, inability to wake normally, new weakness, or a suspected serious neck injury. Emergency evaluation is also needed for repeated vomiting, worsening headache, confusion, slurred speech, unequal pupils, or major balance problems after a head impact.
In an infant or toddler, inability to console them or refusal to nurse or eat after an injury is especially concerning. If a spinal injury is possible, avoid unnecessary movement and follow the emergency dispatcher’s instructions. Do not test the neck’s range of motion yourself.
Symptoms that deserve medical advice
Neck stiffness, headache, irritability, disturbed sleep, dizziness, and reluctance to play can follow an injury. A child may protect the neck, avoid turning toward a sound, or need more help than usual. These changes are not specific enough to diagnose whiplash at home.
Contact the child’s pediatric clinician promptly after an injury when symptoms are present, even without an emergency sign. Explain what happened, when it happened, and what has changed. Seek reassessment if symptoms appear later or are getting worse.
Concussion and neck injury can overlap
A blow or jolt can affect the brain as well as the neck. The clinician may assess alertness, balance, limb strength, tenderness, and other findings. Imaging decisions depend on the circumstances and examination; not every child needs an X-ray or scan.
Remove a child with a possible concussion from sport. They should not return to play that day, and return to school and sport should follow a healthcare professional’s instructions. Pain relief is not the same as medical clearance.
Abusive head trauma is a medical emergency
Violent shaking or a forceful impact can cause severe brain injury in a baby or young child. This is not ordinary whiplash and is not something to treat with a neck adjustment. If shaking or abuse may have occurred, seek emergency medical care immediately.
Never shake a baby. If crying becomes overwhelming, place the baby safely on their back in an empty crib, step away briefly, and ask a trusted adult or healthcare professional for support. Crying alone does not prove an injury or abuse; clinicians need to evaluate the full situation.
Recovery care comes after the diagnosis
Follow the medical team’s guidance on activity, school, pain relief, and follow-up. Do not force stretches, massage an acutely injured neck, or attempt manipulation before serious injury has been excluded. Evidence on pediatric spinal manipulation is limited, and acute trauma requires particular caution.
Use the correct child safety seat and restraint for the child’s size and age, follow sports safety rules, and review return-to-activity instructions with caregivers. These measures reduce risk but cannot make every injury preventable.
Questions about non-urgent follow-up
For Columbus families, Restore’s contact page provides office information for non-urgent questions after medical evaluation. A child’s acute head or neck injury should be assessed by the appropriate pediatric or emergency service first.
